Facial Proportion Science: The Korean V-Line Standard

The V-line face shape is defined by specific anthropometric ratios studied extensively in Korean aesthetic research. Kim and Park (2020) established that the ideal Korean V-line maintains a bigonial-to-bizygomatic width ratio of 0.72-0.78, meaning the jawline should be approximately 72-78% as wide as the cheekbones when viewed from the front. The mandibular angle ideally falls between 120-130 degrees, and the gonial angle should transition smoothly into the chin point without visible angular prominence. Thread lifting addresses these proportions by repositioning the soft tissue envelope along the mandibular border, effectively reducing the apparent bigonial width by 5-8mm through tissue redistribution rather than bone reduction. This soft tissue approach is particularly effective for patients with adequate bone structure but excess soft tissue laxity contributing to a square or round lower face.

Cog Thread Biomechanics for Jawline Vector Control

The V-line thread lift relies on cog threads with bi-directional barbs that create anchoring points within the subcutaneous tissue. Sulamanidze et al. (2011) pioneered the concept of using barbed sutures for facial tissue repositioning and demonstrated that the holding strength of cog threads is directly proportional to the number of barbs engaging tissue and the vector angle relative to the relaxed skin tension lines. For V-line contouring, threads are inserted along three primary vectors: (1) an oblique posterior-to-anterior vector that lifts jowl tissue away from the mandibular border, reducing the squared appearance; (2) a vertical submental vector that tightens the chin-neck angle; and (3) a lateral temporal vector that supports the mid-face component of the V-line silhouette. The interplay between these three vectors, customized for each patient’s anatomy, determines the final V-line shape.

Combination Approach: Threads Plus Masseter Botox

The most effective non-surgical V-line strategy combines thread lifting with masseter botulinum toxin injection, as demonstrated by Lee et al. (2019). Their prospective study of 84 patients showed that the combination group achieved a mean bigonial width reduction of 8.2mm at 3 months, compared to 4.1mm with threads alone and 5.3mm with masseter Botox alone. The synergistic mechanism works on two different tissue planes: cog threads reposition the subcutaneous fat and skin envelope while botulinum toxin induces volume reduction in the masseter muscle itself. (2019) showed that PDO cog threads placed along the mandibular border induced a perifilamentary capsule of organized type I collagen with a mean thickness of 0.8mm at 6 months, which persisted at 0.5mm thickness at 12 months post-insertion — well after complete thread absorption. This collagen scaffold provides ongoing structural support to the V-line contour. Patient satisfaction data from the same study showed 88% of patients rated their V-line result as ‘maintained’ or ‘slightly reduced’ at 12 months. Touch-up treatments with 4-6 cog threads at the 12-month mark can reinforce the collagen scaffold and extend V-line results indefinitely.
